The Texas Medical Disclosure Panel (TMDP) was created in 1977 by the 65th Legislature. The panel, which is comprised of three members licensed to practice law in Texas and six physicians licensed to practice medicine in Texas, is responsible for determining which risks and hazards must be disclosed related to medical care and surgical procedures and creating general forms for the purpose of such disclosure. TMDP has two independent lists, List A and List B, that do and do not require disclosures. Those lists can be found here as well as the general disclosure forms for care and procedures that do require disclosure. You can also find those lists and forms in the Texas Administrative Code here.
