According to CMS, the eligible patient population is Medicare patients with “multiple (two or more) chronic conditions expected to last at least 12 months or until the death of the patient, and for whom the chronic conditions place the patients at significant risk of death, acute exacerbation or decompensation, or functional decline.”
Using 2010 data, a CMS study found that more than two-thirds of Medicare patients had two or more chronic conditions.
Examples of chronic conditions include, but are not limited to, the following:
Alzheimer’s disease and related dementia
Arthritis (osteoarthritis and rheumatoid)
Asthma
Atrial fibrillation
Autism spectrum disorders
Cancer
Chronic Obstructive Pulmonary Disease
Depression
Diabetes
Heart failure
Hyperlipidemia
Hypertension
Ischemic heart disease
Osteoporosis
