Merit-based Incentive Payment System (MIPS) Hardship Exemption Deadline Extended to Mar. 31, 2021
Due to the continuing COVID-19 pandemic, the Centers for Medicaid and Medicare Services (CMS) is automatically applying the MIPS extreme and uncontrollable circumstances policy to ALL MIPS eligible clinicians who do not submit any MIPS data for the 2020 performance period and avoid a 2022 payment penalty. They’re also extending the deadline for groups, virtual groups and Alternative Payment Model (APM) entities to submit an extreme and uncontrollable circumstances exception application through March 31, 2021, to request reweighting for MIPS performance categories. Please note, groups and eligible clinicians who submit data in at least two MIPS categories will override the hardship exception and will be eligible to earn a bonus from the exceptional performance bonus pool, or potentially be subject to a penalty.

If you do not submit your PI performance data by Apr. 1, the hardship exemption will automatically be applied to all eligible clinicians. Once applied, 25% of the PI performance category is reallocated to the Quality performance category.
Groups and eligible clinicians who submit data in at least two MIPS categories will override the hardship exception and will be eligible to earn a bonus from the exceptional performance bonus pool, or potentially be subject to a penalty, CMS said. Otherwise, CMS will automatically apply the hardship exemption for eligible clinicians. In addition, CMS is extending the deadline for groups, virtual groups, and Alternative Payment Model (APM) entities to submit an extreme and uncontrollable circumstances exception application to March 31, to request reweighting for MIPS performance categories.
